Fill out the form below, upload* a copy of the front and back of the camper's insurance card and submit the form. After submitting the form you will need to choose one of the payment options below to pay for your camper. Please print, fill out and sign the
Medical Release form and bring or send to Gilead Baptist Church (our mailing address is on the bottom of the Registration form).
Camp Registration is $90.00 per camper.
If you would rather bring the registration and payment directly to the church feel free to print and fill out the
Registration form.
*If you are not able to scan and upload the insurance card you can make a copy of the front and back and bring/send with the Medical Release form.
***Important: If we do not have the Medical Release form and a copy of the camper's insurance card (if applicable) before camp starts, your child will not be able to go to camp!***
